Lohan was arrested on suspicion of drunk driving after crashing her BMW onto the curb, but was not formally charged because she was seeking medical treatment at a local hospital.
Police claimed a small amount of cocaine was found in the vehicle, but it was not on Lohan's person.
On Thursday, the Mean Girls star surrendered to Beverly Hills police and was formally booked on misdemeanor charges of driving under the influence and hit-and-run, before being released on $30,000 bail.
Lohan is set to appear in Beverly Hills Superior Court on Aug. 24.
Two days after the accident, Lohan checked into Promises rehab center in Malibu, California, where she sought six weeks of treatment. She was released on July 13.
